Finding the causes is essential in accident inquiry because accidents are not random events; they are caused. Any industrial accident should be investigated primarily for the purpose of preventing a repeat of that accident.

Investigations of incidents and accidents are a crucial component of risk and safety management. To avoid accidents and events in the future, disaster learning is essential. Investigations of incidents and accidents offer a deeper understanding of risk and safety management, and discovering what went wrong can enhance risk-control strategies. In order to prevent such occurrences or accidents, it is very helpful to identify their root causes. Investigators need to acquire all relevant information before and throughout the investigation in order to understand the underlying causes of accidents/incidents in an organization/workplace and why and how it happened.

Examining the chain of events that preceded an accident is the first step in determining its cause. The objective of many of the analytical methods used in this discipline is to find this sequence. The investigators next conduct a thorough analysis of every fact to identify the accidents’ underlying causes. They then offer recommendations. This webinar aims to enhance systematic thinking on accident/incidents investigation within enterprises.

“I teach units and supply course materials at EIT. My interaction with the HE team began in February 2020. I graduated from Iran University of Science and Technology (IUST) with a BSc in Railway Engineering in 2005, and Tehran University awarded me an MSc in Civil Engineering – Disaster Management in 2016. I am 40 years old (UT). Tehran University has granted me a Doctor of Business Administration (DBA) degree, which is valid from September 2020 to November 2021.

Since 2004, I have worked in several fields of railway engineering, and I have completed numerous railway projects, including their design and construction.

I have worked with the railway safety sector as a member of the investigative team since I have knowledge in both railroad and safety.

In incident investigation, I’m attempting to establish a systemic picture of safety. This is an event that both seasoned engineers and aspiring engineers should attend to learn more.
